 High Threat Extraction Kit- Introduction
When a casualty is wounded, it is imperative for operational personnel to have the necessary tools available to take immediate action and extract the casualty to a point of cover... This self-contained kit is designed to provide the tactical operator and rescue personnel with all of the essential tools to quickly and efficiently execute rescue operations in a Care-Under-Fire (CUF) high-threat environment. Time and efficiency are the essential keys in the performance of successful rescues. During the execution of a high-threat rescue, equipment must function as an extension of the human body while under extreme circumstances. Immediate casualty extraction becomes the vital link in casualty survival, since tactical casualty care does not take place in exposed areas. Please visit www.NARescue.com for more information.

RAT Strap Extraction Techniques- Feet First
The RAT Strap (Rescue Assault Tether) from North American Rescue, Inc. was designed as a force multiplier during tactical operations. Decreasing man power demands and increasing efficiency of rescuers, the RAT Strap enables hands-free casualty movement and threat suppression simultaneously. Critical to this task is the ability to rapidly detach from casualty depending on dynamic of environment, which is accomplished with the quick release shackle. Ideal for shield-assisted, open-field and indoor-corridor rescues. The innovative Daisy Chain configuration is designed to offer multiple reinforced attachment points accommodating the height of the rescuer and the terrain of the rescue site. Please visit www.NARescue.com for more information.

 Bellevue SWAT Ends Standoff
On July 30, 2007, around 3:00 PM a standoff began between the Bellevue Police Department and an uncooperative man in his Heatherstone apartment, at 15200-15216 NE 8th ST. SWAT units were called in. They deployed some kind of explosive, probably tear gas or concussion grenades, into the apartment. I believe police also cut off electricity and water to the apartment. This event ended safely a little before 7:00 PM. It is unknown to me what the original call was, but a local resident told me that the man had metal problems and did not take his medicine (this is of course unconfirmed).

Crime Scene from Saddle Tree Subdivision 1-04-08
(Thursday, January 3, 2007 @ approximately 8:00 PM) Several Gwinnett County Police officers attempted to serve an arrest warrant last night at 5534 White Cedars Terrace in Buford, which is an area of Gwinnett County. Two police officers were shot when the suspect opened fire on the officers. The surrounding residences were evacuated for public and officer safety. A perimeter was established and Suwanee Dam Road was closed between Whitehead Road and Saltcreek Point. This morning, a nearby elementary school was closed due to the potentially dangerous activity. Again, this was to ensure the public was not at risk. The suspect remained armed in the home as SWAT officers responded to the location. Several attempts to verbally negotiate with the suspect proved futile after seven hours. SWAT continued to use "tear" gas to flush the suspect out of the home. SWAT officers then attempted entry into the home, but were met with gunfire by the suspect. No officers were injured in that encounter. SWAT officers retreated from the home. After another strategic plan was developed by SWAT command officials, the tactical entry team entered the home again around 10:30 AM. Officers found the suspect deceased. It has been determined by the Medical Examiner's Office that the manner of death for Mr. William Caram was suicide. His cause of death was a self-inflicted gunshot wound to the head The officers who were shot are currently in stable condition at Gwinnett Medical Center. The officer who required surgery is a twenty-two year veteran of the department and has served on numerous specialized units during his tenure. He is currently in the Intensive Care Unit, but his condition has been stabilized. The second officer is in stable condition and suffered non-life threatening injuries. He is a seven year veteran with the police department, but has also had previous law enforcement experience in Pennsylvania before moving to Georgia several years ago. There were numerous other law enforcement agencies from the metro area that offered their assistance. On behalf of the Gwinnett County Police Department, we thank you for your support.

 Combat Application Tourniquet®-Application
The C-A-T® is a true one-handed tourniquet proven to be 100% effective by the U.S. Army's Institute of Surgical Research. Tests proved that the C-A-T® completely occluded blood flow of an extremity in the event of a traumatic wound with significant hemorrhage. The C-A-T® utilizes a durable windlass system with a free-moving internal band providing true circumferential pressure to the extremity. Once adequately tightened, bleeding will cease and the windlass is locked into place. A Velcro® strap is then applied, securing the windlass to maintain pressure during casualty evacuation. The C-A-T®'s unique design avoids the use of screws and clips which can become difficult to operate under combat/survival stress or where fine motor skills are compromised. Please visit www.NARescue.com for more information.

 36
 BOA® Constricting Band- Application
One of the best inventions in peripheral IV access. Never before has a constricting IV band been so effective in contributing to successful IV attempts. Once applied to the upper extremity, the BOA®'s revolutionary patented design utilizes a rolling method which transfuses venous blood to the distal veins yet still allows arterial flow. Implementation of this rolling technique distends the veins thus making IV access quicker and more effective even in the most challenging patients. Please visit www.NARescue.com for more information.

 Arm Tourniquet using the SWAT-T
SWAT-T is a new development in bleeding control. In situations where time is critical the SWAT-T provides effective hemostasis in a matter of seconds.

CARE OF CHEST WOUNDS
written by Douglas Kimbell , October 15, 2008

In watching the video on chest wounds, I had concern about one item that appeared. I am working on the assumption that this care is in the civilian law enforecement environment.

The video shows the "medic" tearing the T-shirt to render care. It appears that the "medic" tore the shirt at the hole made by the projectile. I would remind medics that preservation of evidence is vital and you should cut around any holes that may be caused by bullets or knives etc.

Care of Chest Wounds
written by Bill Hauda , October 17, 2008

I agree with Douglas's assessment. Victims of injury are often victims of crimes, and we must try to preserve evidence whenever possible. It should be easy for the medic to cut the clothing in areas away from bullet holes or knife wounds so that the injury to the clothing can be preserved.
